Heart 2 Heart Homecare provides non-medical companion care, personal care and specialized skilled care services to seniors, elderly, mentally challenged and physically challenged adults. Through coordinated care to support our clients and their families, Heart 2 Heart Homecare enables individuals to remain in the safety and comfort of their own home environments for as long as possible. Heart 2 Heart Homecare also serves clients who are transitioning from an assisted living setting or from a hospital, nursing home or rehabilitation facility.
Our home health aides act as a caregiver and friend when family members or loved ones cannot be present. This enables family and friends to have peace of mind knowing that their loved one is getting the attention they deserve, the companionship they desire and the care they need to remain safe in their home.
Our home health aides visit our clients in their homes as often as necessary. For short visits or 24 hours a day. We meet our clients where they are at, and provide the emotional support, caregiver assistance, companionship, and specialized skilled care that they need with love and compassion. Companion Care
Retaining our independence as we age is an important factor in the quality of life. Companion care helps with all basic needs such as grocery shopping, cooking , meal prep, light housekeeping, medication reminders, taking clients to and from appointments, watering plants, taking care of pets or even just someone to visit and chat with. We will pair you or your loved one with a caregiver to fulfill your needs and help you remain as independent as you always have been.
personal care
Personal Care includes everything from Companion Care but is more hands-on. It includes all ADL's such as dressing, medication assistance, bathing, washing hair, shaving, nail care, toileting, transfers, operation of medical equipment such as a c-pap machine, Hoyer’s, sit to stands, shower chairs and power chairs. Personal Care also covers those who need more skilled care for clients who are bedbound, have had a stroke, or have been diagnosed with Alzheimer's or Dementia. We also offer Personal Care for those on Hospice, also known as End-of-Life Care, Cancer Care, Parkinson's Disease and much more.
Alzheimers/dementia care
If you are living or taking care of a loved one with Alzheimer’s & Dementia it can, be very emotionally trying and overwhelming. We understand how important it is to want the absolute best care for your loved one. Care starts with compassion and empathy. People with dementia are prone to becoming confused about their whereabouts and even the time period in which they are living. Dementia is more than memory loss, the symptoms depend on the areas of the brain affected by the disease. Some people may develop difficult behaviors and moods and, in the later stages, can become unable to attend to activities of their daily living independently. They may become non-communicative, unable to recognize loved ones or move about. Heart 2 Heart Homecare offers trained individuals who can meet your loved one where they are at with love, empathy, and understanding to fulfill their daily needs. We also offer 24-hour care rates to assist in lowering the hourly costs associated with around-the-clock care.
Couples Care
We offer a variety of home care services. Couples Care allows the caregiver to take care of two people (generally married couples) in the same household at a discounted rate.
palliative Care
Palliative care is a special kind of care that specializes and focuses on Quality of Life for those with an advanced or life-limiting illness. We strive to provide compassionate and full-hearted care around the clock. We are able to serve you in any way that you need for your family and loved ones to ensure that they may live as comfortably and peacefully as possible. This is where caregiving from the heart matters, and we will provide you with the highest quality of care possible.
Transportation
Our caregivers can assist you with transportation associated with non-medical home care services.
Cancer Care
Cancer can present with an array of symptoms from not feeling well to becoming extremely fatigued and ill. We will provide compassionate and supportive care with trained caregivers to assist you in this time of need. Cancer is both emotionally trying on loved ones as it is the person who is battling it. Care for Veterans
We offer a variety of home care services for everyone. Here at Heart 2 Heart, we appreciate every single one of our veterans and the difference they have made for our country. No matter your level of care or health condition, all veterans receive $2 off per hour.
CHILDHOOD DISABILITY’S
Children with a disability may have special needs, particularly regarding health and education, our caregivers will assist them through their social and environmental barriers in order to fully participate in everyday life in the comfort of their own homes. Some childhood disabilities can include but are not limited to; Down Syndrome, Cerebral Palsy, Fragile X Syndrome, Autism and other disabilities developed after birth or caused by injury or disease. No matter the disability we are here to assist your child in any way he/she needs.
RESpite care
Respite care is short-term care given to families and individuals for both kids and adults with aging or intellectual disabilities. Our caregivers will provide family members and loved ones temporary breaks when it gets to a point where we need a little extra help. Our caregivers will provide any level of personal care service you are needing ranging from companion care to heavy care. The main benefit of seeking respite care is to help reduce the distress of family caregiving. Having help will restore your energy as well as give you time to develop a balance in your life. Taking care of yourself is one of the most important things you can do as a caregiver. Make sure you are making time for yourself, eating healthy foods, and being active. Lately, if you’ve pushed yourself over the edge, respite care can help your mind and body and give you the peace of mind that the one you love is being taken care of properly.
